Required enrollment documents
To complete enrollment, families will need to submit a completed application along with key documents, including proof of age and residency, parent or guardian identification, and any relevant educational records such as report cards, IEPs, or 504 Plans. We also request immunization records or exemption documentation, medical authorization forms, and emergency contact information. If available, diagnostic documentation related to autism, dyslexia, or other learning differences can help us better support your child.
